Select Set Avantra 25 Errors

Our Select Set Avantra 25 with OLP is having a problem when it starts up. During the boot up process it returns the error code 141 hit + to proceed, then error code 143 hit + to proceed, then error code 165 hit + to proceed, then error code 01 appears. Any ideas what might be causing the boot up problem?

Errors generated by Carriage Sensor Driver Module (CSDM board). Located on the top of carriage.
Carriage not driven by pancake motor because DAC failed.
In the best case - just some disconnection at carriage flex cable plug... or one of plugs at CSDM.
In the worst - CSDM failure.

What does DAC stand for?

DAC - Digital-to Analog Converter. Converts signals from Carriage Encoders to proper voltage and current to drive the carriage along the drum's axis (rails) with proper speed and velocity.
