Text to Image? Trapping Woes

Not sure what to make of this problem.

Have a print-ready pdf with 4C + Spot color. The pages have a folio and spot color over a screened black image. Now when run through the trap the folio text traps of some pages and not others. The non-trapped pages when trapped image to image trap fine. After the refine process the folios are turning into images on some pages and not others causing this trapping problem, but when we delete the image there is editable text behind that. (before the refine the text is text and editable.)

Anyone have an idea of what could be causing this? Transparency? We don't have the native files and cannot get them from the customer.

What workflow are you using? It sounds like it is flattening it which will cause the text to convert to image where it touches transparency. If possible keep the transparency live.
